1.1 OBJECTIVES OF THE STUDY
Extracorporeal membrane oxygenation (ECMO) systems are life supports that take
over the function of the lungs or sometimes the heart, outside of a patient’s body. The
procedure is the backbone of the treatment of neonatal, pediatric, and adult patients with
life threatening respiratory and/or cardiac failure.
ECMO is most commonly used in neonatal intensive-care units for newborns with
pulmonary distress. It is around 75% effective in saving the life of such newborns. Today,
technological advances have led to increased research and utilization of this technology
in adult patients with severe respiratory ailments as well. The number of adults for whom
ECMO support is used is increasing dramatically. This was more evident during the
global H1N1 pandemic, which broke out in 2009 to 2011 and tailed to 2012.
ECMO is used in case of respiratory failure, hypoxemia, and pulmonary edema-an
abnormal build-up of fluid in the lungs, Congenital Diaphragmatic Hernia (CDH), and
Primary Pulmonary Hypertension of the New-Born (PPHN)-including idiopathic PPHN,
respiratory distress syndrome, group B streptococcal sepsis, and asphyxia. Although it
is expensive and offered in limited settings, ECMO is likely to be increasingly used for
adults in the coming years. This increase in use can be attributed to the rising incidences
of cardiac diseases as well as the increasing number of cardiac surgeries and heart and
lung transplants performed globally.

There are three modalities of ECMO systems, namely, Veno-arterial (VA), Veno-venous (VV), and Arterio-venous
(AV)-also known as extracorporeal CO2 removal (ECCO2R) system. VA ECMO provides support for both lungs and
the heart. VV ECMO is a type of ECMO that provides support for lungs only. AV ECMO facilitates gas exchange,
especially carbon dioxide removal, by using the patient’s own arterial pressure to pump blood through the circuit.
With this mode of support, oxygenation is provided by slow ventilation of the native lungs, while CO2 removal is
accomplished with the help of ECMO circuit.
