All Foundation members are reminded to send Resolution Health a copy of their latest proof of income before
07 December 2013 to confirm their income bracket for contributions in the coming year. Should we not receive
proof of income from you before 07 December 2013, we will automatically select the 2014 income bracket
above that of your current contribution bracket.
Option changes for 2014
Selecting the correct Resolution Health option plays an important part in ensuring that you have the cover you
need for the coming year. Please familiarise yourself with the various benefit options and enhancements for 2014
and make sure that you select the most appropriate option to suit your individual healthcare needs.
If you wish to change options for 2014, kindly complete the required Option Selection Form (available on
www.resomed.co.za or from one of our call centre agents on 0861 796 6400) as soon as possible and ensure that
